Modeling one puzzle from a puzzle game

A little model of a puzzle game. In the real game, there are 6 * 2^3 = 48 problem configurations, and there are 6 possible answers. The player needs to respond correctly 6 times in a row in order to win.

If the player doesn’t respond correctly, an animation plays explaining (in a somewhat mysterious or mechanical manner), why the answer was not correct, and then the game waits for the player to respond again. This time where the player responds doesn’t matter at all, since their response is a kind of acknowledgement that they have absorbed the explanation, and the game generates a new and completely random challenge.
